Kindergarten Enrollment
Centerville City Schools will hold kindergarten registration for the 2021-22 school year in early March. Registration will take place completely online this year, and information will be available on the district website, www.centerville.k12.oh.us. Students living in the Centerville school district who will be five years old by Sept. 30 are eligible to register for kindergarten.
New Student Enrollment for Grades 1-12

STEP 2: To ensure your child can begin on the first day of school (Aug. 24, 2020), please enroll by Friday, Aug. 14. After completing the enrollment packet and checklist requirements, please print and mail your packet, including copies of any supporting documents listed below, to Centerville Board of Education Office, Attn: Tom Castleman, 111 Virginia Ave., Centerville, OH 45458. You can also fax your packet to (937) 438-6057.

Proof of residency that you reside in Centerville City School district (lease agreement, mortgage papers, property tax bill or copy of deed)
If you are moving into a residence that is registered in someone else's name, they must complete a Residence Affidavit, which must be notarized and brought to your appointment
Official birth certificate or passport
Copy of current immunizations
Custody or shared parenting papers if applicable (official court document)
Copy of your driver’s license
STEP 3: Packets will be processed beginning June 8, 2020. Once your packet is approved, a Centerville City Schools official will be in contact with you.
As we reopen school buildings, we have updated our quarantine guidelines. The Ohio Department of Health still requires school districts to quarantine individuals who have been identified as a close contact; however, the change in quarantine guidance now allows us to quarantine in tiers.
