Please visit our New Student Enrollment page for instructions about how to enroll your children. Please note that your enrollment must be completed by Monday, Aug. 5 to ensure your child can begin the school year on Wednesday, Aug. 14.
What are Returning Student Registration Forms, and how do I access them?
On May 14, parents/guardians with students who are returning to Centerville City Schools received a ParentSquare message with information about updating your child’s Returning Student Data online. These forms must be completed before Aug. 14, 2024.
You can access these forms through the Home Access Center by clicking on the Registration icon at the top of the page and then on the Update Enrollment tab. Look for the 2024-25 Returning Student Data Update. If there is a link to Edit, we still need your child’s information for the upcoming school year. If there is a link to View, that means you have already updated your forms for the upcoming school year.
These forms must be updated electronically and include important information about your child, such as the Emergency Medical Authorization, Guardian and Emergency Contacts, Computer User Agreement, and several others. Please update this information before Aug. 14 so we have accurate information on file for your child.
Please note, if your child is a new student to Centerville City Schools, your family completed these forms during the enrollment process and will not have to complete them again until next year when you will have a returning student.

You can enter your credit card and/or banking information to use with your account or use guest checkout.
In addition to paying student fees, families will also be able to pay fees for athletic and co-curricular activities using PaySchools.
A PaySchools Central app is also available on the Apple App Store or Google Play.
Those who do not wish to pay fees online may submit their payment to their child’s school office. If you have questions about fees, please contact your child’s school.

What transportation forms do I need to complete for my child?
Students at all grade levels must have a Transportation Form on file with their school, whether they are riding a school bus or using alternate transportation. Transportation forms will be sent from each school office via ParentSquare in early June. If you still need to complete your child's transportation form, you can access all the forms from the Transportation page on the district website.
Please note that transportation changes will not be guaranteed between Aug. 7-Sept. 2, and there will be no changes made on Aug. 13-14.
How do I communicate a change in my child’s transportation arrangements?
For long-term changes to transportation arrangements during the school year, please complete a new Transportation Form. You can access these forms from the Transportation page on the district website. If you need to submit a new form during the school year, you must delete your child’s existing transportation form. To do this, open ParentSquare in your web browser, go to My Account, click on Forms, and click on the trash can icon next to the form you want to delete. This cannot be done on the app.
Temporary transportation changes, such as short-term parent pick-up or your child riding a different bus after school, should be communicated to your child's school office. Contact information for each school office can be found on our Contact Us page or by navigating to your child's school on ParentSquare and sending a direct message to the school or unit secretary.
Calendars for transportation are not accepted; you must complete the appropriate form.

How do I add money to my student’s meal account?
Our meal payment portal, TITAN Family Portal, upgraded to LINQ Connect in May 2023. There’s no need to change your username and password -- all your information has already been transferred to the new app.
Visit LINQConnect.com on your browser or download the "LINQ Connect" app from the Apple or Google Play Stores and log in with your existing username and password. If you don’t use our online meal payment portal, LINQ Connect allows you to set up one-time or recurring payments for your student’s meals. You can also view previous transactions, set spending limits, transfer funds between students, set low balance notifications, and more. It’s an easy way to manage meal payments without the hassle of sending cash.
Your child may pay with cash or check through the lunch line. Please make checks out to Centerville City Schools and put your student’s ID and the words "school lunch" on the memo line. If you send in cash or check for more than the lunch price, the money will be applied to your student’s account and every time food is purchased, money will be debited from that balance.
For further questions, please call our Student Nutrition Services Department at 937-433-8841, use the help features on the LINQ Connect website, or email support@linqconnect.com.
How do we apply for free/reduced meals?
Please submit a meal assistance application through LINQConnect.com for the current school year. Click on the “Start Meal Application” button and follow the instructions. Paper applications are available on the Student Nutrition Services website or at your child’s school.
Health & Wellness
- If my child must take medication at school, how should I deliver the medication and care plan to the school nurse?
- What vaccinations should my child have?
- If my child tests positive for COVID-19, do I still need to report it to the school?
- When should I keep my child home from school?

If my child must take medication at school, how should I deliver the medication and care plan to the school nurse?
Please contact the school nurse in your child’s building for further instructions.
What vaccinations should my child have?
All grade levels: Students in all grade levels are required by the Ohio Department of Health to have the required immunizations for school attendance. The state does not require a COVID-19 vaccination at this time.
For incoming seventh graders: If your child will be in the seventh grade this school year, the Ohio Department of Health requires two vaccinations in addition to those your child received prior to school entry: 1) Tdap booster vaccine within the past five years (2018 or later) and 2) Meningococcal vaccine BEFORE the first day of school. If your student has had one dose of this vaccine at any time AFTER age 10 and prior to this school year, this requirement has been met. If you have questions, please contact the school nurse at your child’s middle school. Click here to download the seventh grade vaccination form.
For incoming seniors: Students now must be vaccinated against meningococcal disease (MCV4) prior to entering their 12th grade school year. If the first dose of meningococcal was given on or after the student's 16th birthday, the second dose is not required for his/her senior year. Students who have received these vaccines should immediately turn in their immunization record to the school clinic. Records can be submitted by email, fax, mail or brought to CHS Central Unit or CHS clinic. If you have questions, please contact the school nurse at CHS.
Waivers: The state of Ohio does allow for a parental waiver for those who choose to not vaccinate their children, and a medical waiver for those children who cannot safely be vaccinated due to medical conditions. Click here to download the waiver form.
If my child tests positive for COVID-19, do I still need to report it to the school?
No. The CDC updated its COVID-19 guidance in March 2024 due to the availability of effective vaccines and treatments, as well as population immunity. As a result, COVID-19 health impacts are now increasingly similar to other respiratory viruses.
-
Positive test: Students or staff who test positive for COVID-19, flu, or another respiratory virus must stay home and away from others until they are fever-free (without medication) for at least 24 hours AND their symptoms have improved.
-
Awaiting test results: If awaiting test results for any communicable illness (influenza, COVID, strep, etc.), please stay home until you receive test results.
-
COVID-19 exposure: Students or staff who have been in close contact with an individual who has tested positive for COVID-19 are not required to quarantine and may continue to come to school.
-
Mask guidance: Face masks are OPTIONAL and are not required to be worn inside district facilities or on school transportation for students or staff. We know there may be individuals who prefer to wear a mask, and that choice will be acceptable and supported.
When should I keep my child home from school?
To help prevent the spread of illness in our schools, please keep your child home if they have any symptoms of flu, COVID-19 or any other illness. This could include congestion or runny nose, sore throat, headache, cough, fever or chills, shortness of breath/difficulty breathing, fatigue, muscle or body aches, loss of taste or smell, nausea or vomiting, and/or diarrhea.
Students must be fever, vomit and diarrhea free for 24 hours without the aid of medication before they return to school.
Our school nurses also shared that the American Academy of Pediatrics’ recommends keeping children home from school for the following reasons:
-
If awaiting test results for any communicable illness (influenza, COVID, strep, etc.), please keep your child home until you receive test results.
-
A temperature 100.4 degrees or above is generally recognized as a fever. Whether or not the student should attend with a temperature less than 100.4 degrees should include the student’s overall health status and other symptoms.
-
They cannot participate comfortably in activities and/or pose a significant risk to themselves or others.
-
Their condition results in a need for care that is greater than staff members can provide without compromising the health and safety of other students.
-
They pose a risk of spreading harmful disease to others.
-
Some illnesses/injuries may justify exclusion from school until cleared by a healthcare provider (e.g. a student returning to school after surgery).
How can I help protect my family and the community from illness?
Prevention is a key strategy with any illness, such as influenza or COVID-19, so it is important to both protect yourself and take measures against spread to others. The CDC recommends these everyday actions to help prevent the spread of viruses:
- Wash your hands often with soap and water for at least 20 seconds, especially after touching any frequently used item or surface. If soap and water are not available, use an alcohol-based hand sanitizer.
- Avoid touching your eyes, nose and mouth with unwashed hands.
- Stay home when you are sick. Do not go to work or school.
- Sneeze or cough into a tissue or the inside of your elbow.
- Avoid close contact with people who are sick, especially if you are an older person or someone with a serious underlying health condition.
- Disinfect frequently used items and surfaces as much as possible.
- Get a flu vaccine.
- Pick up your child promptly should they become ill during the school day.
