COVID-19 Reporting

Reporting a Positive Case or Exposure to COVID-19

Students should be screened by a parent/guardian at home, and staff should self-screen for symptoms prior to coming to school. Please stay home if you have symptoms of COVID-19 or any other illness, including congestion or runny nose, sore throat, headache, cough, fever or chills,  shortness of breath/difficulty breathing, fatigue, muscle or body aches, loss of taste or smell, nausea or vomiting, and/or diarrhea.

Any student, staff member or volunteer/visitor who has tested positive for COVID-19 or who has been in close contact with someone with COVID-19 (within 6 feet for a total of 15 minutes or more over a 24-hour period) must complete the following form WITHIN 24 HOURS. Once the form is received, a school nurse or staff member from our Student Services Department will provide isolation and/or quarantine guidance from Public Health - Dayton & Montgomery County.

* District totals include all schools, as well as  staff from the Board Office, Student Nutrition Services and Transportation. Some staff members travel to multiple buildings and may be counted in more than one school's numbers. They are only counted once in district totals.
Reasons for quarantine could include travel, close contact with positive case (within 6 feet for 15 minutes), waiting on COVID-19 test results, etc. Quarantine does not include positive case isolations, which is reflected in the number of positive cases of COVID-19.
For the 2021-22 school year, guidelines for quarantining close contacts has changed. Beginning Aug. 14, 2021, the quarantine numbers will reflect only those individuals who must quarantine outside of school.
