COVID-19 Reporting

Reporting a Positive Case or Exposure to COVID-19

Students should be screened by a parent/guardian at home, and staff will self-screen for symptoms prior to coming to school. Please stay home if you have symptoms of COVID-19 or any other illness.

Any student, staff member or volunteer/visitor who has tested positive for COVID-19 or who has been in close contact with someone with COVID-19 (within 6 feet for at least 15 minutes) must complete the following form WITHIN 24 HOURS. Once the form is received, a school nurse or staff member from our Student Services Department will provide isolation and/or quarantine guidance from Public Health - Dayton & Montgomery County.

* District totals include all schools, as well as  staff from the Board Office, Student Nutrition Services and Transportation. Some staff members travel to multiple buildings and may be counted in more than one school's numbers. They are only counted once in district totals.
Number of confirmed, first-hand positive cases of COVID-19 includes only those that have been reported to the school district.
Reasons for quarantine could include travel, close contact with positive case (within 6 feet for 15 minutes), waiting on COVID-19 test results, etc. Quarantine does not include positive case isolations, which is reflected in the number of confirmed, first-hand positive cases of COVID-19.
