PARENTS OF ALL WATTS MIDDLE SCHOOL INCOMING 7TH GRADERS: TDAP BOOSTER: If your son or daughter will be entering 7th grade in the fall of 2021, he or she MUST have two vaccines required by the Ohio Department of Health
Tdap booster vaccine within the past five years (2015 or later) BEFORE the first day of school 8/18/21.
Meningococcal vaccine one dose BEFORE the first day of school 8/18/21. If your student received this vaccine after the age of 10, the requirement has been met.
These are requirements of the Ohio Department of Health. We know from past years that most of our current 6th graders have had the Tdap booster so all we need is the immunization date. The meningococcal vaccine is a relatively new requirement.
Attached is a form that parents can complete, as well as a listing of immunization options.
We need to have this information on file by the first day of school, August 18, 2021. Please call me at (937) 434-0370 if you have any questions
Thanks for taking care of your child's health needs. Melissa Becker, RN, – Licensed School Nurse
