Back to School Learning Options Deadlines and Links
School begins Monday, August 24, 2020. This is a change from our original school calendar.
You should have already received communication from the district about school learning options via the district email newsletter.
We are asking ALL families to let us know which option they are choosing for each of their children by 11:59 p.m. on Friday, July 24.
You must be registered for the HAC before you can make your learning option selections. Click here if you have not yet set up an account.
Please click here to choose which learning option your child will be doing. Log in using your parent HAC email and password(not the students).
Please click here to choose which blended learning model you prefer if you are choosing the in school option. Hit submit to finish the survey.

ATTENTION PARENTS OF ALL 7TH GRADERS!
If your son or daughter will be entering 7th grade in August, the Ohio Department of Health requires two vaccinations in addition to those your child received prior to school entry. 1. Tdap booster vaccine within the past five years (August of 2015 or later), BEFORE the first day of school. 2. Meningococcal vaccine BEFORE the first day of school. If your student has had one dose of this vaccine at any time AFTER age 10 and prior to August 19, 2020, this requirement has been met. These are requirements of the Ohio Department of Health. We know from past years that many of our sixth graders receive these immunizations at wellness checks and routine appointments. We do NOT need official documentation from the doctor's office, but welcome any copies of immunization records that you provide. Attached is a form that can be completed by the parent or guardian, as well as a listing of immunization options. Please complete this form and submit to the office or clinic. The state of Ohio does allow for a parental waiver for those who choose to not vaccinate their children, and a medical waiver for those children who cannot safely be vaccinated due to medical conditions. Attached is a copy of the waiver form. This information must be on file by the first day of school, August 19, 2020. Please call or email with any questions.
