This is the Transcript Request Form for the Central Unit seniors. You must submit a new form for each college to which your transcript needs to be sent.

To ensure proper processing time, you must allow at least 3 business days prior to your application deadline for each school.

*You are responsible for sending official ACT/SAT test scores directly to the college from the testing organization. These scores are not listed on transcripts.

What type of request is this? *
An INITIAL request is one that is for your first time applying to the school.  A MIDYEAR request is one that occurs as a follow-up to your initial application because the school has requested an updated transcript or you wish to send an updated transcript to show improvements. Midyears will be sent as soon as Sem. 1 grades and GPA and rank calculation are complete. We anticipate this will be in early February.
